As defined by CMS, a problem list is a list of current and active diagnoses along with past diagnoses that is relevant to the current care of the patient (Home – Centers for Medicare & Medicaid Services 2022). It is valuable to have accurate patient problem lists since they are cherished tools for refining the value of care, which enables clinical decision assistance, and aiding investigation and quality measurement (Wright et al., 2012). As explained in the text, the problem list is basically a list of nursing problems to select from to enter in to documentation (Sewell, 2015). Maintaining a medication list helps to keep the practice up to date with current medications since medications are constantly advancing and to help prevent against medication errors. In a recent study on addressing meaningful use and maintaining an accurate medication list in primary care, it was found that even though, medication list accuracy and medication related problems were assessed at the initial and follow up visits, at the scheduled medication reviews with the pharmacists and physicians, did not significantly improve the accuracy of the medication list, however it was associated with fewer drug related problems (Ottney & Koski, 2018).

The benefits of utilizing social media sites for APRNs is to help promote their business if they are running their own clinic for an example, to promote awareness of certain diseases in the community, promote health education, help get the word around when it comes to certain diseases, it is a free or cost-effective method of distributing information to the public, etc. The challenges of utilizing social media sites for APRNs would be making sure to be within HIPAA compliance while doing so or being able to reach the older population where social media could be more of a challenge for them to navigate. Some common social media violations would include: Posting of images and videos of patients without written consent, posting of gossip about patients, posting of any information that could allow an individual to be identified, sharing of photographs or images taken inside a healthcare facility in which patients or PHI are visible, sharing of photos, videos, or text on social media platforms within a private group (Hipaa social media Rules 2022).

Healthcare has transformed itself into a streamlined service aiming to be being more efficient for both patients and providers. While the patient benefits from everchanging resources to promote health, the provider is continuously offered new programs to provide better care. A problem list and medication list are such programs. The federal government requires all electronic health records (EHR) to have a problem list described in the Meaningful Use program (Holmes, 2011). A problem list states all the health problems a patient is facing. In the clinic, the provider can customize care by viewing the list and becoming aware of important health factors. In the community, the problem list can help delineate disease-specific populations via data analysis by finding patients with common illnesses (Holmes, 2011). By doing so, a clinic or research group can create quality improvement measures for that community. Much like a medication list, the problem list allows a provider to keep track of a patients health history without having to rely on their memory. A medication list acts the same way; it lists all the medications a patient is currently taking or has taken in the past. By having such information, a provider can prevent overdosing or prescribing medications that interact negatively with one another. Continually updating these lists keeps the patient from undue harm or negligence on the providers part.

These programs can be easily entered into social media outlets. The rise of social media is far-reaching and inserted itself into healthcare. Advanced practice nurses can utilize social media to benefit their patient care. The benefits of using social media in professional practice empower the patient and allow them to participate and be more active in the treatment decision-making process (Benetoli et al., 2017). Patients feel that they have better relationships with their providers when using social media as a method of communication. However, due to patients increased use of seeking health information online, there is a chance that they come across incorrect or harmful data. The advanced practice nurse cannot control what information their patients choose to retain. When facing a patient who has diagnosed themselves via Dr. Google, the advanced practice nurse then must correct any misconceptions. Another significant barrier towards using social media is the risk of sensitive information being disseminated to the public. Any form of data put into the online world is nearly impossible to take back. Therefore, advanced practice nurses need to be cognizant of what they put online and keep their security breach measures up to date.
