She is a Clinical Director, and her responsibilities are to formulate strategies and plans
for the clinical department, Assign cases, provide supervision to Case Manager or
CSW, assess current mental health programs, restructure existing programs and/or
develop new programs, and overall management of the program under her
supervision. She has been working at the agency for 4 years but has 30 years of
experience in the field.

Provide a concise written summary of the following information from your interview:
(1) What is the mission of the agency or the organization?
To promote the prevention of and recovery from mental illness and substance abuse for the
individuals, families, and communities we serve by providing accessible support, caring, and
culturally competent services.
Citycare provides a variety of services designed to provide the needs of our clients to
improve behavioral health.
(2) Which aspects of social work practice or intervention treatment are evaluated and for what type
of purpose?
Mainly they use a team-based approach to deliver the services and assist consumers in
meeting their needs with supportive services such as medication management psychiatric
evaluation, mental health treatment, behavioral health therapy, housing placement,
employment placement, free transportation, and free meals.
Furthermore, they use many different methods depending on the consumer however their
primary approach is Cognitive behavior techniques.
In a nutshell, they provide medical services, referrals to social services agencies, or linkages
to connect the client to resources in their communities.
Citycare practice supervision, diagnostic/assessment services, counseling,
medication/somatic treatment, substance abuse treatment, psychosocial and psychotherapy
therapeutic techniques, telehealth, and community support services.

When a client is enrolled in our agency through the access helpline, we provide an initial
assessment using Adverse Childhood Experience (ACEs) to get background information in
order to determine the right care and treatment for each consumer.
(3) What types of evaluation methods are being used to evaluate practices, services, or the
intervention-treatments?
Citycare has a quality ensure the team that makes random calls to the client about the
services they are receiving, consumers satisfaction surveys, regular staff meetings, and
ongoing training.
They serve two different programs, a Mental Health Rehabilitation Services MHRS and
Assertive Community Treatment (ACT).
The ACT program is for clients who need intensive services in their communities, and the
program teams meet daily. MHRS consumers are seen once to three times a week, and ACT
consumers are seen more than three times a week, depending on their needs.
(4) Does the agency use an internal or external evaluator for data management and statistical
analysis?
Citycare has our internal compliance team that handles the research of programs and
evaluations. Department of Behavioral Health (DBH) and other community stakeholders are
the external programs that evaluate our data management and statistical analysis. Citycare
are audited by DBH yearly. They are constantly getting feedback on our services on areas
that needs improvement.
(5) What is the client profile involved or targeted by the evaluation, their needs, and frequency of
the evaluations?
Our client population rage from ages 18-99 years old. Citycare work with DC residency who
have mental health disorders.
Based on their diagnostic criteria, most of our consumers present with mental health
disorders such as depression, PTSD, schizophrenia, and anxiety.
As a Core Services Agency, we’ll assess the client and provide the correct diagnosis. Our
treatment plan aims to provide the client with skills and tools that will allow them to remain
independent in the community without being hospitalized.
(6) How, or in what ways, social workers are involved in any evaluation activities?
Social work gives the input regarding whether the client or consumer is complying with
their treatment plan in meeting their goals.
Social workers implement the consumer?s person-centered goals and objectives as
articulated in the treatment plan through appropriate linkages, referrals, coordination, and
follow-up.

(7) What is the interviewee?s reflection on the importance of the evaluation for the agency practices
or program intervention-treatment improvement?
To determine the effectiveness of the services provided to the clients. The evaluation is
critical. It gives us a guideline on our services, where we are, and what needs to be improved
to provide and ensure the best quality services to the community we serve.
(8) Are there any barriers experienced in conducting an evaluation of practices or intervention
treatments?
There are many barriers when providing services to the client. They might be resistant,
hospitalized, incarcerated, in a crisis, or have difficulty even locating and meeting clients.
That can interfere in providing services to the client.
(9) How or in what way is the information from the evaluations being used or applied in the agency
context?
The information on evaluation gives the agency an idea of their progress, what works and
what doesn’t. The compliance team usually assists with the progress report every 6 months
on the effectiveness of the service provided and if there’s a need to change the structure or
utilize a different framework to deliver quality services.
(10) Did anything surprise you during the interview and why or in what way?
The arrival of the pandemic (COVID-19) emergency has led to a global economic, social,
and public health crisis. Specifically, with Mental health, these were some of the challenges
they faced.

? Consumers were able to see multiple people on their treatment team when they arrive at the
office for their appointment
? The office served food such as breakfast and lunch. That brought a good number of
consumers to show up and get services.
? It helped with managing the ability to reduce food insecurities.
? The consumers were consistent with their appointment because they were driven by the
meals that were provided.

Specifically, Citycare is responsible for:
? Preventing all avoidable hospital admissions and readmissions.
? Preventing unnecessary emergency room visits.
? Providing timely transitional follow-up; and
? Decreasing the overall Medicaid cost for District consumers
To be eligible for Citycare services, a consumer shall:
? (a) Be eligible for Medicaid program,
? (b) Be diagnosed as having a serious and persistent mental illness.
? (c) Be enrolled in a CSA or FSMHC.
? (d) Consent to be enrolled in a Citycare Health Services and authorize the disclosure of his
or her mental health, physical health.
? (e) Be at least 18 years of age.

The purpose of care coordination is to increase collaboration and integration of behavioral
health and community-based services, to improve management of chronic conditions, and
reduce avoidable health care costs, specifically for hospital admissions, readmissions, and
emergency room visits.
One of the goals of the Citycare Program is to maintain and improve the health of their
population through the delivery of appropriate services.
