1) I began working in the ED a few months ago. One day, while I was at work, I was assigned to the
Emergency ICU. This is desginated to traumas, stroke alerts etc. On this particular day, we
received a pre notification that a patient was being brought in by EMS as a stroke alert. EMS
alerted us they were 7 minutes out. My preceptor and I would be the primary nurse in this
situation. This was my first stroke alert, so I quickly asked my preceptor (a seasoned nurse) what
to do in this situation. Being that we had the 7 minutes to discuss, he asked me what I knew
about strokes. I went back to my nursing education. I remembered learning “BE FAST”, the
acronym that came in handy that day. Balance, eyes, face, arms, speech and time. My preceptor
aslo taught me to always check the finger stick on a patient with stroke like symptoms, to rule out
hypoglycemia. This was an example of knowledge acquisition, I acquired knowledge from my
schooling combined with new knowledge from a seasoned nurse. Teaching me to check a finger
stick on a patient to rule out hypoglycemia is an example of knowledge dissemination. My
preceptor was disseminating his knowledge on to me. Disseminated knowledge also came into
effect when I was taught the hospitals protocol, place patient on portable monitor and patient
goes to CT. This knowledge will stay with me forever. Another example of how dissemination of
knowledge has been introduced into my career is by listening to the stories of co workers and
what went wrong for them, this becomes a learning experience. McGonigle & Mastrian, 2017
state ?sharing experiences of clinical learning can help convey life-saving information to other
clinicians?.My preceptor is always pushing me and asking me questions. This is an example of
generating and processing knowledge. My preceptor often asks me ?what are we going to do first
with this patient?, ?which patient will you see first? ?what are you worried about with a patient
involved in a MVA?? This is beneficial in allowing me to think critically and use my knowledge to
perform optimal patient care.
Two informatics competencies that are essential for an entry-level nurse are basic technology/computer
skills and informatics skills. Having the right technology skills is crucial in the health care system. Nurses
must be able to navigate medical records and programs that are pertinent to the patient in which they are
caring for. It is often through technology the nurses and providers can communicate to each other about
any given patients conditions. Nurses may also use programs to communicate with the pharmacy, which
is also very important in delivering patient care. Self-Assessment of Nursing Informatics Competencies in
Hospitals (2021) stresses the importance of information technology as a critical part of health care
delivery.

2) I work in Interventional Radiology which involves minimally invasive procedures performed
under Xray guidance. Prior to these procedures, I am responsible for interviewing the patient
and obtaining a full medical history to identify risk factors. When I was interviewing Mrs. S, I
obtained her vital signs and performed a finger stick because she is diabetic. Her blood sugar
reading was 58mg/dl. Although asymptomatic, I proceeded to give her 4 glucose tablets which
totaled 15 grams of glucose. I spoke with her doctor and he said to continue to monitor her
blood sugar every 15 minutes. It went up to 92mg/dl which was sufficient. The reason I gave
glucose tablets was to prevent a hypoglycemic episode which is based of my previously acquired
knowledge from nursing school. I used transparent wisdom to assist my patient and provide her
with fast and effective care (McGonigle & Mastrian,2017).
In this situation, knowledge acquisition was obtained by recognizing that her blood glucose level
was below normal standards, which I learned from my nursing education. Knowledge
dissemination was performed by educating the patient about checking blood sugar often and
knowing when to administer insulin and when to eat her meals. Knowledge generation was
obtained because my patient was asymptomatic which made me realize that hypoglycemia will
not always present as a patient being cool and clammy, she was alert and stable. Knowledge
processing was obtained by performing a glucose check with the proper items handy.
Two informatic competencies that are essential for entry level nurses are basic computer skills
and navigating a patient?s EMR. I was able to check my patient?s previous blood sugar levels
because I am knowledgeable in EMR. It allowed me to see that this patient has had
asymptomatic low blood sugars in the past which led me to properly educate her about this
dangerous issue. According to an study performed by Mohamadian & Farzandipour, nurses had
a better understanding of obtaining key information in patient?s charts rather than backing up
computer software.
